Any person with a terminal illness, regardless of age, is eligible for our hospice care programs. Home Hospice Care comes to you, wherever you reside–your home, assisted living residence, or skilled nursing facility.

Any person with a terminal illness, regardless of their age, is eligible for our hospice care programs. Many different diagnoses may be considered, including Alzheimer’s and related dementias, pulmonary and cardiac diseases and cancer. Hospice care is a covered benefit under Medicare, Medicaid and most insurances. Hospice care comes to you, wherever you reside – your home, assisted living residence or skilled nursing facility.
With the Regional Hospice team, you can expect an individualized care plan. Each patient’s personal needs for pain management and symptom control will be closely considered in order to provide the best hospice experience as possible.
Hospice care is not just for the comfort of the patient – it is also for family and any caretakers. You and your loved ones can expect emotional and spiritual support throughout this difficult time. Whenever your caretakers have any questions regarding patient needs, our staff and nurses will provide the help and answers you need.
When there is a terminal prognosis of 6 months or less, home hospice care may be right for you. However, the right time to begin hospice care is personal and can only truly be determined by you, your family and your physician. We encourage you to discuss hospice care with your physician and ask for a referral if you feel it is right for you.
Anyone can make a referral – a patient, family member, physician or friend
Your hospice care services will include an individualized care plan developed in conjunction with the Regional Hospice team that meets each patient’s personal needs for pain management and symptom control. Our caring team members and trained volunteers available to visit you where you live including Fairfield, Litchfield and New Haven.
Home hospice care involves skilled nursing/assisted living personnel under contract with Regional Hospice. We offer clinical support at our Danbury facility 24 hours a day, 7 days a week. We also cover:
